Dear Non-UA/UPH Physicians:

The Office of Graduate Medical Education and the Department of Obstetrics and Gynecology here at the University of Arizona College of Medicine would like to thank you for your interest in serving as an attending physician for resident physicians in our program. The relationship between our residents and community physicians is possible, in part, because of a contractual agreement between the University and affiliated hospitals.

To provide an option that would permit our residents to participate in a clinical setting with community physicians, we have preceptor agreements that articulate the separate responsibilities of the UA and the preceptor. This agreement also spells out the direct supervisory relationship between that particular physician (and his/her group) and the resident. Please download the request form from the link below; returned to me at the above address. I will then forward the form to Judy McElroy in the Contracts Office and she can prepare a Preceptor Agreement per your request.
